DESCRIPTION:YoloCares is offering a monthly Advance Care Planning Workshop for SNAHC patients\, using the Five Wishes workbook to help participants reflect on their values and make informed decisions about future healthcare in a way that honors Native traditions.\n\n\nThe workshop takes place on the 2nd Tuesday of each month from 12:00 to 1:30 p.m.\, with odd months hosted at the Florin site and even months at the J Street site.
