2024 Community Needs Assessment

The Sacramento Native American Health Center, Inc. (SNAHC) is excited to share its 2024 Community Needs Assessment (CNA), which can be accessed using the links below.  CNAs are a way to get a sense of the health priorities of the community, and SNAHC conducts them every 3 years to be sure our services are in line with the population in Sacramento County. CNAs are often conducted using surveys, focus groups, interviews, or other pieces of information from patient records. SNAHC utilized surveys for our patients and focus groups such as American Indian/Alaska Native Youth, Grant Union High school youth for our School-Based Health Center, and Native Community members for our Traditional Health services.

Thank you to all our amazing community members and patients who participated. We will use these findings to continue our vision of a vibrant community built upon a strong foundation of cultures and traditions, where healthy lifestyles include the balance of mind, body and spirit – a restored legacy of well individuals and families.
