Thank you for your interest in SNAHC. To receive care, please call (916) 341-0575 to make an appointment with our Member Services team. A member services team member will help you complete all your registration paperwork. This appointment will take approximately 30 minutes. Once complete, you will be able to schedule an appointment with our clinical departments.

Preparing for your Appointment

Please plan to arrive at least 15 minutes before your scheduled appointment. Routine appointments should be made with as much advance notice as possible to ensure we can work with your schedule. Although we do make same-day appointments according to availability, it is always best to call ahead to determine whether we can see you or not. If you have not completed initial registration, or you are in need of an annual update, you will not be eligible for a clinical appointment until the registration or update is completed.

No Show Policy

Patients will be required to cancel their appointment 2 hours prior to their start time or they will be considered a no-show. No-show appointments prevent other patients from accessing valuable services. Three no-shows will trigger a re-engagement process. If a patient no shows again, after the re-engagement process, this could result in being removed as a SNAHC patient of record. If you have any questions, please email
