ALL ARE WELCOME
If you need to see a doctor, dentist or other health care practitioner, we can help. There are no tribal or ethnic requirements to receive care at SNAHC—we are open to everyone. If you are uninsured or have trouble paying for service, talk to us. We will help you determine how your health care needs can be met.
To begin care, please call (916) 341-0575 to make an appointment with our Member Services team. A member services team member will help you complete all your registration paperwork. This appointment will take approximately 30 minutes. Once complete, you will be able to schedule an appointment with our clinical departments. Please bring the following to your Member Services appointment:
- Medical or Dental insurance card(s)
- MediCare card
- Current, Valid State-Issued Photo ID
- Tribal enrollment documents
- Birth Certificates for any minor children
- Immunization records for any minor children
- Proof of Income

In accordance with Affordable Care Act (ACA) requirements, we need to assess your eligibility for insurances. It is important that you, and each member of your family, are covered by stable health insurance. If you are not currently enrolled in any form of insurance, a SNAHC representative would like to work with you to determine your eligibility and to help you enroll. SNAHC accepts a variety of public and private insurances. Most patients qualify for a public insurance and we will work with you until we have exhausted every resource.
