SNAHC provides Adolescent Health services via a School-based Health Center on the campus of Grant Union High School. This location, called Pacers Take Space, is the result of a unique community-based collaboration between Neighborhood Wellness Foundation, Twin Rivers Unified School District, Sacramento Native American Health Center, and Sutter Health. The collaboration was borne out of a desire to directly address the student needs identified in a 2017-2018 University of California, Davis School of Nursing Student Needs Assessment which identified the growing mental and physical health needs of teens in the Del Paso Heights region of Sacramento County. The on-campus services are available at no cost to Grant Union High School students.

- Health check-ups
- Vaccinations
- Sports physicals and exams
- Health education
- Reproductive and sexual health services
- Medi-Cal enrollment support
