WHAT IS THE COMMUNITY ASSESSMENT PANEL?
The Sacramento Native American Health Center, Inc. (SNAHC) strives for the best health care and environment for patients, families, and community members through collaboration with health home members (i.e., active patients) and parents of health home members. To this end, the SNAHC Board of Directors established the Community Assessment Panel (CAP) with the intent of reviewing and assessing SNAHC’s services using this collaborative process. The goal of the CAP is to convene a group of health home members and parents of health home members committed to improving SNAHC service delivery and patient experience. The CAP reviews matters relating to patient services, programs, outreach and education. The panel also supports point-in-time projects or initiatives requiring patient feedback and SNAHC outreach events.
Frequently Asked Questions
How many people can be on the Panel and for how long?
Nine to twelve people may participate at any one time. Members are appointed for a one year term, and participate for three consecutive terms.
When and how often does the Panel meet?
The Panel meets once a month, through Zoom or in person.
Why do members have to have a background check and medical clearance?
All agents of the health center, including volunteers, are subject to background clearance checks for criminal history in order to protect patients’ safety. Additionally, we require screening for tuberculosis so as to prevent exposure.
