Enhanced Care Management, or ECM, is a Medi-Cal benefit that offers extra support for patients who are managing complex health needs or difficult life situations. If healthcare feels overwhelming, you do not have to handle it alone.
ECM connects you with a dedicated Case Manager who works with you, your doctor, and your care team to help you stay healthy and supported.
Who ECM is For
ECM is available for patients with Medi-Cal enrolled in one of the following health plans:
ECM may be a good fit if you:
- Have ongoing health conditions that are hard to manage
- Have been to the emergency room or hospital several times
- Are dealing with housing, food, transportation, or other daily challenges
- Need help keeping appointments or understanding your care
- Feel overwhelmed navigating healthcare and community services
Your provider or care team can help determine if ECM is right for you.
How ECM Can Help
Your ECM Case Manager can support you with:
- Scheduling and keeping medical appointments
- Understanding your health conditions and medications
- Coordinating care between doctors, specialists, and services
- Finding help with housing, food, transportation, or other resources
- Following up after hospital or emergency room visits
- Setting and working toward health goals that matter to you
Support is personalized to your needs and may change over time.
What to Expect
If you join ECM:
- You will be matched with a Case Manager who gets to know you
- They will check in with you regularly
- They will help create a care plan based on your goals and needs
- They will work with your healthcare team to make sure everyone is on the same page
You stay in control of your care. Participation is voluntary, and you can stop at any time.
Language Support
We want you to feel comfortable communicating with your care team.
- Spanish-speaking Case Managers are available at our Midtown clinic
- Interpretation services are available for other languages at no cost to you
