Please complete the following form to pre-register. Each participant must submit their own registration form and will be notified once their registration is confirmed. Additional registration information may be collected on the day of the event for first-time attendees.

This workshop is limited to 15 participants. Only individuals who are registered will be able to participate. If all spaces are filled, individuals may be placed on a waitlist and contacted if space becomes available.

Health Education Class Cancellations and No-Show Policy: A person who misses their health education class appointment without canceling at least 24 hours prior to the appointment start time is considered a no-show. Participants who no-show three times will be required to sign the Health Education Class No-Show/Re-engagement Letter before RSVP’ing to or attending another next health education class. Participants who no-show four times may be discharged from all SNAHC health education classes.
